1. About RewireOT

RewireOT is a therapy clinic that offers both in-clinic and virtual services. Our licensed professionals provide occupational therapy and related care to help clients achieve their wellness goals. RewireOT is also a FND resource website that helps share resources, publications and support understanding of the FND diagnosis and treatment. All views on this website are that of RewireOT and/or shared from other sources to which credit will be given.

2. Services Offered

We provide:

• In-Clinic Occupational Therapy at our physical location

• Virtual Occupational Therapy Sessions via secure, HIPAA-compliant video platforms. Virtual treatment currently available to Missouri Residents.

All services are subject to availability and professional discretion. Access to scheduling assessments and treatment is available through RewireOT.Janeapp.com

3. Eligibility

To use our services, you must:

• For youth clients have parental consent and filled out appropriate intake forms with this consent provided for treatment

• Young adults/adults have filled out intake forms and provided accurate medical information to initiate treatment.

• Reside in a region where we are licensed to provide care (Missouri for Telemedicine)

• Provide accurate and complete personal and health information (youth, young adult and adult clients)

4. Appointments & Cancellations

• Appointments can be booked online or by contacting our clinic through RewireOT.com.

• We require at least 24 hours’ notice for cancellations.

• 3 or more missed appointments or late cancellations will initiate a conversation regarding appropriateness and readiness for treatment. A subsequent personalized plan will be initiated to optimize care.

• RewireOT reserves the right to reschedule or cancel sessions due to emergencies or provider availability.

5. Telemedicine Terms

By using our telemedicine services, you agree to:

• Ensure a private, quiet, and safe environment for your session

• Use a reliable internet connection and compatible device

• Not record sessions without prior written consent

Telemedicine may not be suitable for all conditions. We reserve the right to recommend in-person care when necessary to personalize and optimize care.

6. Fees & Payment

• Fees are communicated at the time of booking.

• Payment is due at the time of service unless otherwise arranged.

• We accept credit card payments after each treatment and can provide a superbill upon request to submit to your insurance to request reimbursement.

• Clients are responsible for all charges.

• If the client or caregivers are not able to make payments, please discuss with RewireOT owner Jason Kreuzman to discuss potential scholarship opportunities or support.

7. Privacy & Confidentiality

We are committed to protecting your privacy. All personal and health information is handled in accordance with HIPAA and applicable privacy laws. For more details, please review our Privacy Policy located on RewireOT.janeapp.com .

8. Client Responsibilities

Clients agree to:

• Arrive on time and prepared for sessions

• Communicate openly and respectfully with staff

• Use telehealth services in a distraction-free setting

We reserve the right to discontinue services for inappropriate behavior or misuse.

9. Limitation of Liability

RewireOT is not liable for:

• Technical issues during telemedicine sessions

• Delays or disruptions beyond our control

• Decisions made based on incomplete or misrepresented information

10. Changes to These Terms

We may update these Terms from time to time. Continued use of our services constitutes acceptance of any changes.
